Frailty Care Coordinator Team

What is frailty?

If someone is living with frailty, it doesn’t mean they lack capacity or are incapable of living a full and independent life. When used properly, it describes someone's overall resilience and how this relates to their chance to recover quickly following health problems. It can affect people of all ages, often those with long term conditions.

What is a Frailty Care Coordinator?

Our Frailty care coordinator team holistically support patients living with frailty within the community. We work closely with our patients to help support and manage their health and wellbeing to continue to help our patients live independently in their own homes for as long as possible.

Our role is to be a point of contact for both patients and their families and carers, whilst working alongside other health, social care and voluntary agencies.

How can a Frailty Care Coordinator help me?

We as Frailty Care Coordinators will be a single point of contact for you and your families/carers.

Some of the support you can expect from our team includes:

• Reviewing your needs and help you access the services and the support you require to understand and manage your own health and well being
• Refer you to services within the area that are most appropriate for you
• To assist in the formulation and review of care plans to ensure all care is reflecting your needs and wishes, future treatment, next of kin etc.

If you are eligible for this service, over the coming months you may be contacted by one of our team, where we will arrange an appointment to talk with you or your family/carer either by telephone or in your home (We are unable at this time to take self-referrals to the service)

We may feel that one appointment is sufficient, however if you have more longstanding issues, we will support you whilst the issues are resolved.
